  • Wonderful Resource

As a parent of a child with a behavioral/mood disorder, the episode on depression management was a vital educational resource. It gave us a lot of important insights and questions to ask our providers in our search for how best to help our child.

  • Turning Tragedy into Triumph

What a heartbreaking and powerful podcast. Dr. Sherlock’s story was so moving. Not many people could have endured and overcome the unimaginable physical and emotional traumas that she did, and how she has dedicated her life to caring for babies and helping their families cope during their experiences in the NICU. It was heartwarming to hear that she was able to be a mother after losing her firstborn. Thank you for this episode.

  • A promise fulfilled

What a wonderful story of deep friendship and generosity. It brought tears to my eyes and filled my heart with joy. With so much sadness in the world-illness, weather-related problems, mass murders, anger, etc, this story proves that there is goodness in the world. Thanks for doing this podcast, and wishing both doctors continued good health.

Talking Points

Recent interactions between the hosts and their guests.

Thyroid Disorders in Kids
Q: How urgent is it to refer a child with suspected Graves' disease to endocrinology?
Graves' disease suspected with clinical symptoms and lab results indicating hyperthyroidism should be referred urgently, typically with an undetectable TSH and elevated T4.
Thyroid Disorders in Kids
Q: What should a pediatrician do if a thyroid nodule is palpated on exam?
If a palpable nodule or asymmetry is found, start with a thyroid ultrasound and obtain thyroid labs (TSH and free T4) as part of the initial workup; avoid routine screening ultrasounds in the absence of a palpable abnormality due to high incidental finding rates.
Thyroid Disorders in Kids
Q: You've described primary hypothyroidism as most commonly autoimmune. Could you elaborate on how to approach screening in a child with growth concerns?
Screening should include both TSH and a free T4 when evaluating growth, since central hypothyroidism would show a low T4 with an inappropriate TSH response. If growth is an issue but TSH is normal, consider broader endocrine evaluation and monitor growth trajectories closely.
The Parent Behind the Patient
Q: What advice would you give to clinicians about communicating with families who are anxious or resistant?
Be compassionate and patient, read the room, adjust your tone, and be willing to have tough conversations while staying on the same team, always validating the caregiver's feelings and offering practical steps like sleep improvement strategies or referrals when needed.
Coordinating Pediatric Aerodigestive Care
Q: How would you describe the perfect aerodigestive patient to a primary care provider?
A patient with interrelated breathing, swallowing, and growth issues who would benefit from a coordinated visit with multiple team members in the same room to develop a unified plan that aligns with the child's overall health goals and family needs.
