Overall wonderful updates. Listen to these while driving. Would however appreciate if the Oncology nursing series was on a separate platform or podcasts. When one is driving the podcasts roll on and I don’t want to hear the nursing discussions .It is somewhat dangerous to be messing with the podcasts while driving. (With all due respect to nursing collegues). Also in areas where there is no reception, manually skipping a talk often just causes the connection to drop.

Simply one of the best most complete and informative podcasts in any medical topic ever made. I am a pediatrician and do not treat cancer. Yet I listen to every new episode. Outstanding.

HER2-Positive Metastatic Breast Cancer — An Interview with Prof Dr Sibylle Loibl on First-Line and Maintenance Therapy
Q: How long should maintenance continue after six cycles of T-DXd, and is there randomized data to guide stopping points?
Maintenance decisions should be made with patients, as no randomized data clearly define stopping after six cycles; some centers consider continuing while tolerating, and future maintenance studies may guide a switch to other regimens, but current practice varies and centers tailor to patient tolerance and preferences.
HER2-Positive Metastatic Breast Cancer — An Interview with Prof Dr Sibylle Loibl on First-Line and Maintenance Therapy
Q: In what situations would you not use T-DXd in first-line therapy?
The main limiting factors are frailty, age-related considerations, and significant preexisting pulmonary disease; otherwise, many patients without severe comorbidities were included in trials, favoring broader use with careful monitoring.
Breast Cancer — Proceedings from a Session Held in Conjunction with the 2026 ASCO Annual Meeting on the Use of Oral SERDs and Agents Targeting the PI3K/AKT/mTOR Pathway
Q: What is your view on using gerodestrant in adjuvant settings if approved, and how would you sequence it with CDK4/6 inhibitors in higher-risk patients?
The consensus acknowledged gerodestrant's potential, particularly in intolerant patients or those needing long-term endocrine therapy alternatives, but emphasized that sequencing with CDK4/6 inhibitors would be tailored to risk level and tolerability, with a preference to defer until CDK4/6 inhibitors have been used in higher-risk patients unless data strongly support earlier use.
Breast Cancer — Proceedings from a Session Held in Conjunction with the 2026 ASCO Annual Meeting on the Use of Oral SERDs and Agents Targeting the PI3K/AKT/mTOR Pathway
Q: Would you use a CERD monotherapy or a CERD plus targeted agent in ESR1-mutant patients who progressed after CDK4-6 inhibitors?
Many panelists favored a CERD-containing combination in ESR1-mutant patients with prior CDK4/6 inhibitors to address cross-resistance, though some remained cautious about monotherapy in certain contexts; the discussion highlighted ebalance between efficacy and toxicity, and the potential sequencing of CERDs with mTOR/PI3K inhibitors.
Breast Cancer — Proceedings from a Session Held in Conjunction with the 2026 ASCO Annual Meeting on the Use of Oral SERDs and Agents Targeting the PI3K/AKT/mTOR Pathway
Q: What first-line treatment would you recommend for a 65-year-old woman with HR-positive HER2-low metastatic breast cancer who develops metastatic disease after AI therapy and has a PI3K mutation?
The panel favored a combination approach in many scenarios, often preferring triplets or CERD-containing regimens in endocrine-resistant settings, especially if there is a PI3K mutation; however, decisions were individualized based on mutation status, prior therapies, and tolerability, with some favoring triplets like fulvestrant plus CDK4/6 inhibitors with a PI3K inhibitor when feasible.
