Overall wonderful updates. Listen to these while driving. Would however appreciate if the Oncology nursing series was on a separate platform or podcasts. When one is driving the podcasts roll on and I don’t want to hear the nursing discussions .It is somewhat dangerous to be messing with the podcasts while driving. (With all due respect to nursing collegues). Also in areas where there is no reception, manually skipping a talk often just causes the connection to drop.

Simply one of the best most complete and informative podcasts in any medical topic ever made. I am a pediatrician and do not treat cancer. Yet I listen to every new episode. Outstanding.

Talking Points

Recent interactions between the hosts and their guests.

Triple-Negative Breast Cancer — Fifth Annual National General Medical Oncology Summit
Q: How do you approach ER-low tumors in the metastatic setting, and does IO + ADC strategy apply there?
ER-low tumors are treated with IO-based strategies in some contexts, acknowledging that biology is ongoingly refined; decisions often depend on BRCA status and PD-L1, with a growing interest in integrating IO with ADCs in first-line settings.
Triple-Negative Breast Cancer — Fifth Annual National General Medical Oncology Summit
Q: Any thoughts on how to best sequence ADCs in TNBC given limited post-ADC options?
Clinicians acknowledge limited effective therapies after ADCs and tend to use sequential ADCs or switch to single-agent chemotherapy or clinical trials, with decisions guided by patient performance status and prior responses.
Desmoid Tumors — Microlearning Activity 3 with Prof Bernd Kasper
Q: What is the consensus on how long to treat desmoid tumors with medical therapy, particularly gamma-secretase inhibitors?
The consensus from the Desmoid Tumor Working Group suggests treating for at least six to twelve months before evaluating effectiveness, acknowledging that data for optimal duration are limited and that some patients may benefit from longer therapy if tolerated and responsive.
Desmoid Tumors — Microlearning Activity 3 with Prof Bernd Kasper
Q: Can you talk a little bit about Abdominal Wall Desmo and how the algorithm differs from other localizations?
The abdominal wall scenario is common in younger patients and is treated with a mix of surgical and medical approaches, though surgery is increasingly de-emphasized in favor of systemic therapy and, when appropriate, local ablative techniques; the algorithm for abdominal wall emphasizes when intervention is needed and how to balance risks with non-surgical options.
Desmoid Tumors — Microlearning Activity 2 with Dr Ravin Ratan
Q: When do you decide to stop therapy, and what duration do you typically target?
There is no universal stopping rule; many clinicians consider stopping after about two years if there is stable disease and good tolerance, with six months to a year as a practical starting point for treatment duration and regular reassessment for each patient.
