Overall wonderful updates. Listen to these while driving. Would however appreciate if the Oncology nursing series was on a separate platform or podcasts. When one is driving the podcasts roll on and I don’t want to hear the nursing discussions .It is somewhat dangerous to be messing with the podcasts while driving. (With all due respect to nursing collegues). Also in areas where there is no reception, manually skipping a talk often just causes the connection to drop.

Simply one of the best most complete and informative podcasts in any medical topic ever made. I am a pediatrician and do not treat cancer. Yet I listen to every new episode. Outstanding.

Talking Points

Recent interactions between the hosts and their guests.

Desmoid Tumors — Microlearning Activity 2 with Dr Ravin Ratan
Q: When do you decide to stop therapy, and what duration do you typically target?
There is no universal stopping rule; many clinicians consider stopping after about two years if there is stable disease and good tolerance, with six months to a year as a practical starting point for treatment duration and regular reassessment for each patient.
Desmoid Tumors — Microlearning Activity 1 with Dr Ravin Ratan
Q: What is the current stance on surgery for desmoid tumors in 2026 based on the literature discussed?
Surgery is increasingly avoided for most desmoid tumors outside of special circumstances or emergencies, with ongoing debate about location-specific indications and significant caution in FAP-associated mesenteric disease due to wound-healing and recurrence concerns.
Desmoid Tumors — Microlearning Activity 1 with Dr Ravin Ratan
Q: What were the safety signals observed with prolonged Nirogasistat exposure, particularly regarding ovarian function?
We observed irregular periods and changes in reproductive hormones indicating ovarian toxicity, which appeared to be reversible after stopping treatment, though fertility impact could not be definitively ruled out and counseling with fertility experts was recommended.
Desmoid Tumors — Microlearning Activity 1 with Dr Ravin Ratan
Q: Could you review the initial efficacy data in terms of response rate and duration of responses?
The DEFY trial showed a 41% response rate with Nirogasistat versus 8% with placebo, and the median PFS in the treatment group had not been reached at the time of publication, indicating meaningful activity but with ongoing uncertainty about long-term durability.
Diabetology in Breast Cancer — Managing Hyperglycemia in Patients with Breast Cancer Receiving Agents Targeting the PI3K/AKT/PTEN Pathway
Q: How should baseline HbA1c be considered when starting PI3K inhibitors like Capivasertib or Inavolisib?
Baseline HbA1c should guide risk stratification; even patients with acceptable HbA1c can experience rapid glucose elevations during therapy, so a plan that includes metformin, GLP-1 agonists, and close monitoring is recommended, with insulin reserved for severe cases or poor control.
