  • BT is an asset to IR

This podcast provides a valuable service to Interventional Radiology. As someone who has practiced in a diversity of settings from premier academic, to private practice, to OBL. I feel that this podcast has valuable content for every practice setting.

You can listen to specific episodes on procedures you are developing and it will definitely help you get started and get better. Listening to episodes on procedures I am not involved in has also re-stimulated my curiosity and love for IR.

  • Episode 381 Anesthesia

Another outstanding episode and discussion about sedation and escalation to anesthesia. Collaboration with anesthesia and building their resources into your IR platform improves access and effectiveness. It’s more than an anesthesia machine. Getting their Omnicell and proper gas access into the room while formalizing escalation strategy has been effective at our institution. Bringing anesthesia in has drastically improved our patient experience and also has elevated our ability to do more advanc... more

Ep. 625 Managing Acute Arterial Thrombosis: Devices & Approaches with Dr. Shang Loh and Dr. Khanjan Nagarsheth
Q: How do you decide between Artix, Pounce, or other devices in a given case?
Decisions are case-specific, relying on lesion length, presence of chronic disease, vessel size, and whether you need to maintain wire access; we often start with aspiration to create a channel, then deploy a basket-type device if needed, balancing the risk of distal embolization and the need to minimize invasiveness.
Ep. 625 Managing Acute Arterial Thrombosis: Devices & Approaches with Dr. Shang Loh and Dr. Khanjan Nagarsheth
Q: Is lysis dead, or still used in certain scenarios?
Lysis remains useful for particular outflow issues or when there is heavy calcification or chronic disease that hinders mechanical approaches; we use lysis selectively, often for outflow when inflow has been addressed and there are still residual thrombus.
Ep. 625 Managing Acute Arterial Thrombosis: Devices & Approaches with Dr. Shang Loh and Dr. Khanjan Nagarsheth
Q: What are the newer devices available beyond traditional thrombectomy catheters, and how do they change your lab workflow?
The newer devices, like Artix and Pounce, offer a dual mechanism with aspiration and mechanical thrombectomy, which allows us to debulk and retrieve clot in a single session more reliably; this has reduced ICU dependency and enabled one-stage procedures in many cases.
Ep. 624 Integrating Spinal Cord Stimulation in Vascular Disease Management for CLTI with Dr. Mary Costantino and Jill Sommerset
Q: What data or evidence is needed to advance this therapy?
More prospective data, standardized ultrasound measures of perfusion, trials sponsored or funded to study outcomes, and broader education across specialties to reduce barriers and improve patient selection.
Ep. 624 Integrating Spinal Cord Stimulation in Vascular Disease Management for CLTI with Dr. Mary Costantino and Jill Sommerset
Q: What were the practical hurdles to adopting SCS in this setting?
Site-of-service reimbursement, ASC vs OBL models, neurology involvement for implantable devices, and coordinating across wound care, podiatry, and vascular teams were major barriers that needed addressing.
