    This podcast provides a valuable service to Interventional Radiology. As someone who has practiced in a diversity of settings from premier academic, to private practice, to OBL. I feel that this podcast has valuable content for every practice setting.

    You can listen to specific episodes on procedures you are developing and it will definitely help you get started and get better. Listening to episodes on procedures I am not involved in has also re-stimulated my curiosity and love for IR.

Activity is the amount of radioactive material ordered; dose is the energy delivered per unit mass, and they are related but not interchangeable. The choice of activity and how it distributes in tissue determines the final dose.

Q: So, where do you fall in the line of multiple radiation segmentectomies versus a low bar dose with partition dosimetry?

Q: Can you describe the transhepatic approach and when you choose it over other options?
The transhepatic approach is chosen when there are no patent iliac veins and the occlusion extends to the central veins. We access a hepatic vein under ultrasound guidance, advance through the liver capsule with a fine needle, and transition to a sheath and snare system to recanalize from above while maintaining a safe working path and protecting the azygos inflow.

Q: What are your go-to techniques and tools for crossing a central venous occlusion, and when do you decide to switch to advanced methods?
We begin with ultrasound guidance, standard catheter and guidewire techniques, using a back-up approach via the femoral vein if the initial wire fails. If crossing fails, we escalate to dual access, snare techniques, and eventually transepatic or transhepatic routes with specialized needles and longer sheaths, staying mindful of the stenosis length and the surrounding structures.

Q: Walk us through your initial workup for a patient needing central venous recanalization.
We start with a CT venogram to map the venous anatomy and identify complete exhaustions of conventional access. If jugular, subclavian, and iliac routes are unavailable, we proceed to a dual-access plan and consider transepatic or transhepatic approaches as needed, always confirming anatomy with imaging before proceeding.
