  • Improve the audio!

Please improve the audio quality! I really like listening but end up turning up the volume to try and hear the people that sound like they are murmuring, and then having to turn it down when another person chimes in. I struggle to hear most of it and then feel deafened!

  • Great education and conversation. Need better sound quality.

I enjoy the lively, relaxed discussion from the presenters about important topics relating to Acute Medicine and look forward to new updates. I can relate to their honesty when they are stumped with a clinical dilemma. My only gripe is the sound quality which sounds like it is done via online conference call and is not as good as other podcasts. My suggestion is that the audio be done via a landline if possible or maybe better microphones for the preseters?

Talking Points

Recent interactions between the hosts and their guests.

Acute Cardiology - Part 2
Q: What is the current thinking on the rise and fall of troponin, and does it depend on the assay used?
Assay-specific algorithms exist; generally, a normal start with at least a 50% rise to enter the positive range is a typical rule, while if the troponin is already positive, a 20% rise or fall is often clinically significant, with zero- to three-hour sampling being standard.
Hospital at Home
Q: What are the practical enablers for starting a Hospital at Home program?
Key enablers include strong hospital infrastructure (diagnostics, imaging, IV therapy, EHR integration), a capable multidisciplinary team (nursing, pharmacy, biochemistry), reliable remote consultation networks, and leadership backing to overcome cultural and organizational barriers.
Hospital at Home
Q: What evidence supports cost-effectiveness and patient outcomes for Hospital at Home?
Randomized trials, including NIHR-supported studies, showed equivalent clinical outcomes to hospital care while enabling cost savings over time, particularly when long-term effects like reduced functional decline and delayed need for care home placement are considered.
Hospital at Home
Q: How did Hospital at Home evolve from ambulatory care and what sparked the transition to home-based care?
The transition grew from recognizing that many acutely ill patients could be managed safely at home with targeted diagnostic testing, IV therapy when needed, and close monitoring, which reduces hospital admissions without compromising outcomes. A pragmatic shift toward treating patients where they are—at home or at the front door of the hospital—proved feasible and beneficial.
Opthalmology
Q: What would you do for a patient with a sudden visual loss?
Clarify if it's monocular or hemianopic loss and assess the duration of symptoms, as this will guide further management and potential treatments like a paracentesis.
