Talking Points

Recent interactions between the hosts and their guests.

ACMT Highlights Episode 72: Legal and Ethical Considerations for Toxicologists at Mass Gatherings
Q: You discuss several federal laws that impact approaches to harm reduction at mass gatherings. Can you describe these laws a little bit?
Key points include the Federal Analog Act extending scheduling to analogs of controlled substances, the distinction between structural and functional analogs, explicit exceptions for substances that are already scheduled, FDA-approved studies with NDAs, and labeling not intended for human consumption; together these shape what organizers and toxicologists can credibly implement at events.
ACMT Highlights Episode 69: CBRNE Training in Ukraine
Q: You talked about multiple simultaneous program streams that were implemented in response to the war in Ukraine. Can you review these with us as well as some of the complexities in bringing education to Ukraine?
The team ran several concurrent courses, including CBRN education alongside ATLS, Trauma Nursing Fundamentals, Pre-Hospital Trauma Fundamentals, Pediatric Trauma Fundamentals, a WHO Mass Casualty Management course, and Stop the Bleed, with Ukrainian colleagues deeply involved in adaptation, translation, and ensuring relevance on the front lines. Equipment and training had to be simple, scalable, and cross-border friendly, especially given supply chain disruptions from COVID and the invasion. We learned to prioritize low-resource, reusable setups and to engage local partners to sustain the programs.
ACMT Highlights Episode 68: Harm Reduction - Rapid Inpatient Methadone Initiations in the Fentanyl Era
Q: What does the literature say about rapid inpatient methadone titration versus symptom-inhibiting fentanyl induction?
Both strategies aim to rapidly reach a therapeutic methadone dose in hospitalized patients, with inpatient titration having a larger literature base; symptom-inhibiting induction can achieve rapid stabilization but has limited supporting studies, suggesting a need for careful weighing of risks and benefits.
ACMT Highlights Episode 68: Harm Reduction - Rapid Inpatient Methadone Initiations in the Fentanyl Era
Q: You talked about three key features of Fentanyl. Can you describe these?
Fentanyl has very high affinity for the mu-opioid receptor, is highly lipophilic allowing rapid penetration of the blood-brain barrier, and exhibits high efficacy that drives potent downstream effects, all contributing to extreme opioid tolerance.
ACMT Highlights Episode 67: Psychedelics and Palliative Medicine
Q: What challenges do you anticipate in the inclusion of psychedelic medicines in approaches to palliative care?
The biggest challenge is that these are all still federally illegal, impacting the ability to prescribe and research these treatments.
