Q: Gopa, can you walk us through the Nexus trial data and explain what it suggests about sequencing after EV therapy and the potential for nectin-4 targets after EVP?
The Nexus trial shows a nectin-4–targeted ADC with a topo-1 payload has activity post-EVP, particularly in EV-naive and post-EVP populations, but responses are lower in post-EVP patients who had prior topoisomerase exposure, indicating payload resistance plays a role and sequencing matters.

Q: So let me jump in with the question. In your practice then, if a patient achieves CR, are you using that as a time point to stop therapy, continue for two more cycles, or do you not really necessarily account for that and you're just basing it on toxicity?
My approach is evolving. In patients with liver metastases and a CR, I'm more reluctant to stop therapy, while in node-only disease I'm more comfortable discussing discontinuation, but many patients are not eager to stop; the decision is guided by ongoing toxicity, quality of life, and biomarker trends like ctDNA.

Q: How do you view the clinical significance of an MFS hazard ratio around 0.8 in a disease with relatively favorable long-term outcomes?
While 0.8 is not overwhelming, it translates into meaningful delay of metastasis and potential deferral of additional therapies, particularly important for younger patients who may live with the disease for many years; it should be considered alongside safety and patient preference when evaluating practice changes.
