Q: Gregg, before diving into the depth of microbial specificities, can you please explain briefly to our listeners in which step are we on in the study of microbiome in inflammatory diseases?
We are in a formative stage with momentum built over years; technologies now allow us to count and interpret gut microbial communities, and we're starting to link specific bacterial expansions to distinct immune-activation patterns, which may explain why some patients progress differently.

Q: When would you consider starting immunotherapy in a young patient with several organ systems affected and a significant interferon signature?
Therapy should be guided by the most active organ manifestations and overall disease activity, with a preference for approaches that address the current dominant manifestations (e.g., nephritis, myositis, or joint disease) rather than chasing a single biomarker, and considering safety and pregnancy plans.
Challenging Cases: Haematological Lupus and the "Coagulation Tightrope"
Q: Ricard, can you just briefly tell a little bit more about the CAHPS registry, and did it help us to better understand a little bit more about the catastrophic antiphospholipid syndrome?
The CAHPS registry has demonstrated to be a very useful tool to know better on this rare condition, because less than 1% of patients with the antiphospholipid syndrome develop this catastrophic variant.
