Talking Points

Recent interactions between the hosts and their guests.

Peripheral Blood Gene Expression and Scleroderma Lung Disease
Q: What is the practical takeaway for clinical decision-making or trial design moving forward?
Peripheral gene expression profiling may help stratify enrollment or guide monitoring in SSC-ILD trials, especially when background immunosuppression is allowed, though it should complement, not replace, clinical markers like CRP.
Peripheral Blood Gene Expression and Scleroderma Lung Disease
Q: Why was the Senscis trial's placebo arm, and its design, particularly valuable for this analysis?
Senscis provided a large, international, standardized dataset where half the placebo participants were on background mycophenolate and half were not, allowing us to separate natural history from treatment effects and to validate findings across trials.
Peripheral Blood Gene Expression and Scleroderma Lung Disease
Q: Could you orient us to the context of this study and the central questions you aimed to answer?
We aimed to determine whether peripheral blood gene expression modules could predict ILD progression and treatment response, and whether these signals were prognostic or predictive by analyzing data from a large randomized trial with a true placebo arm.
Fecal Metabolome in Early Systemic Sclerosis
Q: From a patient perspective, how should we interpret these findings and can diet influence these factors?
There is evidence suggesting diet may impact the gut microbiome and metabolome, as seen in related research linking ultra-processed foods to dysbiosis and symptoms. While more work is needed to translate findings into dietary guidelines, reducing processed foods and incorporating healthy fats may support a healthier metabolome and gut environment.
Fecal Metabolome in Early Systemic Sclerosis
Q: Tell us how you designed this study and who you recruited.
The study used a Swedish cohort of early systemic sclerosis patients (disease duration ≤3 years) and matched healthy Swedish controls, focusing on treatment-naive individuals to minimize confounding factors. They collected stool samples for metabolomic analysis and linked findings to gut microbiome profiles, with plans to validate results in blood and in additional cohorts.
