Talking Points

Recent interactions between the hosts and their guests.

Ep. 19: W. Norman Scott, MD and Mark W. Pagnano, MD
Q: After you got into medicine, why orthopedic surgery?
He explains his love for sports, basketball in particular, and how that affinity naturally drew him toward orthopedic surgery, eventually aligning with the Knicks as their team doctor for many years.
Ep. 19: W. Norman Scott, MD and Mark W. Pagnano, MD
Q: Well, let's start way back, maybe at the beginning of your career. How is it that you decided to pursue a career in medicine?
Norman attributes his path to a great-aunt who was a physician and encouraged him, describing how a mix of personal encouragement and early exposure steered him toward medicine, followed by a strong interest in sports which eventually led him to orthopedics and knee surgery.
ZB Value-Based Care: Ep 1
Q: How have you measured success in value-based care within your programs?
Success is measured through patient-reported outcomes (PROMs), infection and readmission rates, length of stay, and the overall patient experience. The teams described achieving high PROM capture rates (up to around 90%), using digital platforms integrated with their EHRs, and applying data to optimize pathways and reduce variations in care.
ZB Value-Based Care: Ep 1
Q: What does value-based care mean in your practice settings, and how have you approached it historically?
Value-based care, in our discussion, is an equation where outcomes are divided by cost, with a focus on improving outcomes and the patient experience while reducing unnecessary costs and length of stay. Academic and private practices both emphasized the shift from volume to value, with ongoing learning from early bundles like BPCI and CJR shaping current approaches.
Ep. 23: Thomas K. Fehring, MD and William G. Hamilton, MD
Q: What were the key takeaways from the trial, and do you think single-stage is becoming the standard of care?
The study showed higher early success with one-stage, sustained at two years, and Fehring notes that strict protocol adherence is essential; while promising, he emphasizes that practice patterns will take time to shift and that the protocol must be followed to realize benefits.
