  • Great but please stop using AI

The podcast is a great quick knowledge refresher or a way to stay up to date. I’ve not listened for a while due to being on Mat Leave, but now the podcast is read by an AI voice and it is so difficult to listen to and actually understand due to the unnatural voice tone and breaks. Please go back to a human reading.

Talking Points

Recent interactions between the hosts and their guests.

Difficult-to-Treat Depression: First, Second, and Third-Line Strategies
Q: What can we do for the pharmacologic management of difficult to treat depression?
Begin with a diagnostic recheck, assess adherence and interactions, then optimize the current antidepressant dose and consider augmentation with first-line agents like aripiprazole or brexpiprazole, while also incorporating psychological treatments; if needed, move to adjuncts or third-line options with careful attention to side effects and functioning.
Xylazine in the ED: Managing Toxicity
Q: In such a scenario, do you have xylazine intoxication or a mixed xylazine opioid intoxication on your differential?
Dr. Gorelick explains that many cases involve a co-intoxicant like fentanyl, so treating the opioid component with naloxone is reasonable, but the management must be tailored to supportive care since xylazine itself is not an opioid.
Could Lithium Play a Role in Preventing Alzheimer's Disease?
Q: So, what is lithium orotate?
Dr. Beach explains that lithium orotate is different from the lithium forms used for mood disorders, potentially avoiding sequestration by amyloid plaques and thereby staying in the brain where it's needed; however, human trials are not yet completed and regulation of supplements is a concern.
Predicting Lithium Response in Bipolar Disorder
Q: What does the latest precision medicine work suggest about predicting who will respond to which bipolar treatments?
It shows that beyond traditional guidelines, individual clinical features and family history can guide choices, and that non-drug strategies or different augmentations may outperform simply increasing antidepressant doses, though differences in efficacy across strategies were not always statistically large.
Preventing Relapse: The Role of LAIs in Schizophrenia
Q: What about our 28-year-old patient who struggles with adherence and relapses?
Based on the evidence and the patient profile, this case meets multiple criteria for LAI candidacy, reinforcing the need to discuss LAIs with patients.
