Talking Points

Recent interactions between the hosts and their guests.

Enhanced Recovery Protocol for Children Undergoing Surgery
Q: What were the major clinical findings regarding outcomes and fidelity to the protocol?
We observed a modest increase in protocol elements delivered per patient across phases, and importantly, when patients received 13 or more elements, they showed shorter hospital stays and lower opioid exposure, even though the overall length of stay did not significantly change when looking at phase-based comparisons alone.
Enhanced Recovery Protocol for Children Undergoing Surgery
Q: Can you walk us through a little bit about how you did this study?
We designed a prospective type 2 hybrid implementation and effectiveness study with stepped-wedge, cluster randomization across 18 hospitals, which allowed us to evaluate both how well the protocol was implemented and its clinical effects across a diverse pediatric population.
Small Bites vs Large Bites for Closure of Abdominal Midline Incisions
Q: Were there any surprising aspects of the long-term results?
The durability of the effect was striking, with early divergence in outcomes that persisted for over a decade, and the observation that even when hernias occurred in the small-bites group, they tended to be much smaller and less clinically burdensome.
Small Bites vs Large Bites for Closure of Abdominal Midline Incisions
Q: Can you summarize the main long-term findings of the trial and what they mean for surgical practice?
The 13-year follow-up showed a clear and durable reduction in incisional hernias with the small-bites technique compared to large-bites, with smaller hernias and improved quality of life in the small-bites group, suggesting that meticulous, small-bite closure should be preferred in midline laparotomies.
Data and the Art of Surgical Preference Card Maintenance
Q: How should surgeons go about optimizing preference cards at their own hospital?
Surgeons should collaborate with local data scientists to analyze their preference cards and surgical receipts, ensuring they understand what is being used and what can be removed.
