  • Great family med shelf review podcast

Excellent free podcast that covers many high yield topics for the family med shelf exam. Love the practice question format, the broad range of topics, and the host’s delivery. I just found this podcast (one week into my fam med clerkship) and am very grateful for the hours of valuable study content to listen to while driving, at the gym, doing chores, etc. Many thanks to the creator(s)!

#52 - Endocrine Disorders
Q: In a patient with hyperprolactinemia after excluding pregnancy and hypothyroidism, what is the next best step in management?
Imaging with a pituitary MRI to evaluate for prolactinoma and determine tumor size before initiating dopamine agonist therapy.
#54 - Gallbladder & Biliary Disorders
Q: Question 4. A 39-year-old man with ulcerative colitis has elevated alkaline phosphatase and MRCP showing multifocal biliary strictures. Which complication is the greatest risk to develop?
Cholangiocarcinoma.
#54 - Gallbladder & Biliary Disorders
Q: Question 3. A 62-year-old man presents with fever, jaundice, RUQ pain, hypotension, and confusion. RUQ ultrasound shows gallstones and CBD dilation. What is the most appropriate next step?
IV antibiotics and urgent ERCP for biliary decompression.
#54 - Gallbladder & Biliary Disorders
Q: Question 2. A 45-year-old woman presents with persistent RUQ pain, fever, nausea, vomiting, and a positive Murphy sign. RUQ ultrasound shows gallstones but is inconclusive. Which is the most appropriate next step?
Hidascan (HIDA scan). Non-diagnostic ultrasound with high clinical suspicion for acute cholecystitis warrants a Hidascan showing cystic duct obstruction.
#54 - Gallbladder & Biliary Disorders
Q: Question 1. A 42-year-old woman presents with several recurrent episodes of right upper quadrant abdominal pain occurring shortly after eating fatty meals. The pain remains steady for about two hours and then resolves completely. She is afebrile, and her WBC, bilirubin, and liver enzymes are normal. RUQ ultrasound shows gallstones without gallbladder wall thickening or peri-cholecystic fluid. Which of the following is the most appropriate next step in management?
Elective laparoscopic cholecystectomy. This is symptomatic cholelithiasis with biliary colic; recurrent stones are definitively treated with elective cholecystectomy.
