The model concentrates on three pillars—improved access, physician-led teams, and ongoing quality improvement. We've deployed nurses to take on many routine tasks, implemented same-day triage with 811-based protocols, and used flow sheets to guide care, which has shortened visits and increased overall patient throughput.

It's been incredibly rewarding and at times challenging, but the core has always been the relationships with patients and the ability to help them across different life stages; the role has evolved with changes in technology and care delivery, yet the essence of continuity and belonging remains central.
