The Latest in PBC with Gideon Hirschfield
Q: What do you think the most important unmet needs are?
It's a mixture. It's number one, I'd say, recognizing that there's an unmet need in terms of still preventing cirrhosis, which is where targeting disease control.

Q: As your trial has helped create, there's a brave new world of multiple approved second-line agents suddenly for PBC. When you do need to start a second-line agent, are you mainly turning towards a PPAR agonist or are there still cases in which you're using a beta-colic acid?
There will be an adjustment in the choice of second-line therapies. It is fair to say that if you're a patient and there's a drug that might make you itchy, I'm not saying that beta-colic acid makes you itchy all the time.

Q: When you're starting someone on a versatile that's newly diagnosed with PBC, how long do you give it before starting to think about a second-line agent?
You know after six months you're going to have a pretty good idea. Everyone deserves the best care, but some patients have greater needs.
The Latest in PBC with Gideon Hirschfield
Q: What sparked your interest in that particular field?
It fits the way I think. I like to think about how patients present and how that then drives the question how to make a difference for patients.

Q: Can you describe for our listeners the scope of your current practice and how you came to be a hepatologist?
I'm very lucky that I have a practice that's become completely focused on autoimmune liver disease and other rare liver diseases.
