The team started with typical management for acute cholestasis with suspected cholangitis, performed EOS and ERCP, and only later, after recurrent symptoms and new imaging findings, identified a vascular source (pseudoaneurysm) as the cause of the ongoing problems.

We position the patient in the left lateral decubitus position and perform under MAC anesthesia, avoiding intubation unless absolutely necessary, to balance safety with procedural access.

The patient had a benign, refractory stricture that failed multiple conventional dilations with steroids, and the team concluded that adding spray cryotherapy could provide a more durable mucosal disruption and healing response, allowing the patient to come off dependence on NJ feeds.
