I really like this podcast. I disagree with many reviewers that it’s short. It’s the perfect length for listening on a walk or a commute or a break. If you want an hour-long plus podcast then you should be checking out the website or journal.

  • Called Lite for a reason; the previous ACCEL was so much better

I love this podcast, but I miss the breadth and depth of interviews that I used to hear on the original ACCEL/Audio Digest. I felt I could keep up with new developments and thinking by listening to the 3 hours of interviews each month, but now ACCEL is much more limited, thus “Lite.”

  • Superficial

Compared to previous version of ACCEL this is very superficial and shallow . I miss the old ACCEL with detailed interviews and pdfs . CME was nice too . I would not mind paying for the ACCEL if it is detailed and has CME included. The way it was in the past

Talking Points

Recent interactions between the hosts and their guests.

Evaluating the 2025 AHA/ACC High Blood Pressure Guideline
Q: Can you talk to us for a couple of minutes about the history of hypertension?
The history shows a shift in thinking from fixed targets to recognizing blood pressure as a continuous variable, and an evolving understanding that even mild elevations can contribute to organ dysfunction over time, guiding a more nuanced approach to treatment thresholds.
The Power of Nutrition: GOFRESH Trial
Q: So what were the results, Stephen?
The GOFRESH Trial showed good engagement and a clear difference in blood pressure reductions between groups: the grocery intervention reduced systolic pressure more substantially than the stipend, and there were meaningful improvements in LDL cholesterol as well; however, the benefits diminished during the maintenance phase when groceries were withdrawn, highlighting the importance of ongoing access.
OCEAN Trial: Antithrombotic Therapy after Successful Catheter Ablation for Atrial Fibrillation
Q: How should clinicians apply these findings in practice?
For patients more than a year post-ablation who are clinically well and have CHADS-VASc scores of 1–3, consider discussing withdrawal of anticoagulation, while recognizing that those with a recent stroke remain high risk and may still need ongoing anticoagulation.
OCEAN Trial: Antithrombotic Therapy after Successful Catheter Ablation for Atrial Fibrillation
Q: What were the main results regarding safety and efficacy?
Event rates for stroke or embolism were exceptionally low in both groups, with only five events in the anticoagulation arm and nine in the aspirin arm over three years, and major bleeding not different between groups; minor bleeding was more common with rivaroxaban.
OCEAN Trial: Antithrombotic Therapy after Successful Catheter Ablation for Atrial Fibrillation
Q: Could you just briefly describe the design of the study and how you define the endpoints?
We enrolled patients with various AF patterns who were at least one year post-ablation with documented rhythm stability and CHADS-VASc criteria, then randomized them to rivaroxaban or aspirin, followed for three years with brain MRIs to detect large embolic events; the primary endpoint was a composite including any large covert strokes.
