I really like this podcast. I disagree with many reviewers that it’s short. It’s the perfect length for listening on a walk or a commute or a break. If you want an hour-long plus podcast then you should be checking out the website or journal.

  • Called Lite for a reason; the previous ACCEL was so much better

I love this podcast, but I miss the breadth and depth of interviews that I used to hear on the original ACCEL/Audio Digest. I felt I could keep up with new developments and thinking by listening to the 3 hours of interviews each month, but now ACCEL is much more limited, thus “Lite.”

  • Superficial

Compared to previous version of ACCEL this is very superficial and shallow . I miss the old ACCEL with detailed interviews and pdfs . CME was nice too . I would not mind paying for the ACCEL if it is detailed and has CME included. The way it was in the past

Timothy Wong's August 2024 presentation on HCM. Dr Wong refers to LVH as a symptom. LVH is a sign or a finding, but it is not a symptom.

Talking Points

Recent interactions between the hosts and their guests.

A Look At The ORBITA-CTO Trial
Q: What were the eligibility criteria for patients in the study?
Eligible patients had single-vessel CTO with demonstrable ischemia and viability and a JCTO score of three or less, ensuring that only those likely to benefit from angioplasty were included.
A Look At The ORBITA-CTO Trial
Q: How was the placebo environment created to ensure blinding?
We recreated the cath lab experience with intravenous sedation, auditory isolation, clock removal, and staff changes to keep patients blind to their treatment, and we verified blinding with a blinding index showing successful concealment.
A Look At The ORBITA-CTO Trial
Q: What was the starting point for this trial today?
We moved from the original ORBITA results through ORBITA 2 and then focused on ORBITA CTO to determine if CTO angioplasty offers an incremental benefit beyond placebo in symptomatic single-vessel CTO; the key finding is a significant reduction in angina burden with PCI in the right patient group.
Lower LDL-C for Longer? Defining the Optimal Timing and Intensity of Rx
Q: So what is your approach to managing these patients? And in particular, how do you go about implementing optimal timing and intensity of treatment?
Dr. Blumenthal emphasizes early, sustained LDL reduction starting in midlife or earlier, using high-intensity statins first and adding ezetimibe or PCSK9 inhibitors when needed to reach target LDL levels, while balancing cost and patient risk factors.
Keeping it Simple: Top 10 Things to Know About the 2026 Dyslipidemia Guideline
Q: With the prevent calculator now recommended and the observed underestimation of risk compared with PCE, how should clinicians adapt their approach, especially when EGFR data is incomplete or unavailable?
Clinicians can approximate by using a reasonable EGFR assumption when data is missing, and perform parallel estimates to assess whether preventive actions should be initiated earlier. The goal is to balance accuracy with practicality, recognizing that prevent generally yields lower risk estimates and may prompt earlier intervention in higher-risk individuals.
