I really like this podcast. I disagree with many reviewers that it’s short. It’s the perfect length for listening on a walk or a commute or a break. If you want an hour-long plus podcast then you should be checking out the website or journal.

  • Called Lite for a reason; the previous ACCEL was so much better

I love this podcast, but I miss the breadth and depth of interviews that I used to hear on the original ACCEL/Audio Digest. I felt I could keep up with new developments and thinking by listening to the 3 hours of interviews each month, but now ACCEL is much more limited, thus “Lite.”

  • Superficial

Compared to previous version of ACCEL this is very superficial and shallow . I miss the old ACCEL with detailed interviews and pdfs . CME was nice too . I would not mind paying for the ACCEL if it is detailed and has CME included. The way it was in the past

Timothy Wong's August 2024 presentation on HCM. Dr Wong refers to LVH as a symptom. LVH is a sign or a finding, but it is not a symptom.

Talking Points

Recent interactions between the hosts and their guests.

Heart and Liver Health: Interconnectedness of CV and Hepatic Risk
Q: What practical steps should a clinician take after finding a high fibrosis score?
Refer to hepatology for advanced liver imaging or biopsy as needed, to confirm fibrosis stage and inform coordinated care that may improve both liver and heart outcomes.
Heart and Liver Health: Interconnectedness of CV and Hepatic Risk
Q: Should clinicians screen HFpEF patients for liver disease, and how can this be done cost-effectively?
Yes, implement noninvasive fibrosis risk scores (NFS and FIB-4) using readily available demographics and labs, and use tools like MDCalc to guide further hepatology referral when fibrosis risk is high.
Heart and Liver Health: Interconnectedness of CV and Hepatic Risk
Q: Is that a real association between fatty liver disease and heart failure, or is it just shared risk factors?
The association appears real, with fatty liver disease contributing to higher risk for cardiovascular conditions through mechanisms like visceral adiposity and inflammation, rather than being solely due to common risk factors like obesity.
Prehospital Heparin Administration In Patients With STEMI Undergoing Primary PCI
Q: What is the big advantage of pre-hospital Heparin administration in STEMI patients undergoing primary PCI?
Giving Heparin before angiography increases the likelihood that the infarct-related artery is open at the time of the procedure, without adding significant bleeding risk, due to its short action and the availability of an antidote if needed.
Definitions and Treatment of Iron Deficiency in HF: Feel and Function or More?
Q: How do you diagnose iron deficiency in heart failure independent of hemoglobin?
Use ferritin thresholds and transferrin saturation: ferritin under 100, or ferritin 100–300 with TSAT under 20%, to define iron deficiency.
