Talking Points

Recent interactions between the hosts and their guests.

#145 REPOST: Bronchiectasis
Q: How would you go about investigating a patient with bronchiectasis?
Order a high-resolution CT scan to confirm bronchiectasis, followed by blood tests including autoimmune screens, immunoglobulin levels, sputum cultures to identify pathogens, and lung function testing to assess obstruction or restriction, with bronchoscopy as a rare consideration if necessary.
#141 Funky SOB
Q: What makes hypersensitivity pneumonitis distinct in the history?
Focus on organic exposures such as birds, molds, damp environments, and hobbies that may lead to exposure; ask about pets and molds in the home.
#141 Funky SOB
Q: How do you assess occupational exposure in these patients?
You explore current and past jobs, specific tasks, exposures to asbestos, silica, woodworking, and home environment, then connect exposures with symptom patterns.
#141 Funky SOB
Q: Which drugs are most important to screen for in drug-induced ILD, and how do you structure that history?
Key drugs include nitrofurantoin, amiodarone, methotrexate, and recent immunotherapies; you map symptom onset to drug exposure and review past uses in detail.
#141 Funky SOB
Q: Could you outline your approach to history-taking for a patient with exertional breathlessness?
The emphasis is on timing (when it happens), trajectory, triggers, and then expanding into exposures, medications, and occupational history to differentiate ILD, COPD, and other causes.
