Key operative pearls include being precise with skin excision, being aggressive with scar extension when needed, and being careful about nipple positioning considering the patient's individual goals.

Q: Do you think that when you use nerve allograft versus direct co-optation that there's a difference in operative time?

In my experience, both techniques do not vary much in time, though direct nerve co-optation requires longer dissection of the nerve.

I have adopted a systematic approach to TNR, initially using nerve allografts and transitioning to direct coaptation when feasible after becoming comfortable with identifying nerve branches.
