  • JACC specialty podcast

JACC specialty podcasts are getting more numerous and higher in quality. This requires a lot of work in time, so the efforts are commendable. Podcasts need to focus on objective evidence, and hyperbole and opinions should be minimized. Listeners want to have the evidence and the actual data, not broad subjective assessments of treatments and interventions.

The Future of Cardio-Oncology: Progress and Priorities | ACC.26 | JACC: CardioOncology
Q: Is myocarditis all the same disease, or are there subtypes we need to phenotype and treat differently?
The guest acknowledges significant uncertainty about subtypes and calls for research to identify distinct phenotypes, which would inform tailored treatment strategies and better outcomes.
The Future of Cardio-Oncology: Progress and Priorities | ACC.26 | JACC: CardioOncology
Q: One is the right ventricle is often the bottom child in cardiology. How can we best try and tackle this in the future?
The guest suggests that we do not fully know the incidence or best imaging practices yet, and highlights the need for more robust data, standardized imaging protocols, and targeted research to understand RV-specific risks and management.
TRANScatheter edge-to-edge rePAir for SAM related mitral REgurgitation in patieNTs with HCM. The TRANSPARENT registry | JACC: Cardiovascular Interventions
Q: What were the main procedural outcomes and early clinical results?
Among 35 patients treated with MitraGloPhy devices, technical success reached 94 percent, with 30-day device success around 91 percent and overall procedural success about 88 percent. Post-procedure MR was no or trivial in about 90 percent of patients, and the LVOT gradient fell substantially, indicating a favorable acute hemodynamic effect.
TRANScatheter edge-to-edge rePAir for SAM related mitral REgurgitation in patieNTs with HCM. The TRANSPARENT registry | JACC: Cardiovascular Interventions
Q: What were the inclusion criteria for registry patients?
Patients had known hypertrophic cardiomyopathy with a peak LVOT gradient above 30 mmHg at rest or during provocative maneuvers, mitral regurgitation linked to SAM rather than primary leaflet disease, and a high surgical risk where other options were not feasible; all were at least NYHA class III and previously deemed unsuitable for surgery.
TRANScatheter edge-to-edge rePAir for SAM related mitral REgurgitation in patieNTs with HCM. The TRANSPARENT registry | JACC: Cardiovascular Interventions
Q: What motivated the creation of the Transparent Registry and what patient population were you targeting?
The registry was born out of an unmet clinical need in inoperable or high-risk hypertrophic cardiomyopathy patients with SAM-related mitral regurgitation, where conventional therapies were limited. We aimed to collect real-world data from multiple centers to evaluate whether a transcatheter edge-to-edge repair could reduce SAM motion and MR, potentially improving outcomes for this challenging group.
