Antibody approaches like Rituximab deplete B cells broadly and may trigger collateral inflammation or miss CD20-negative plasma cells; CAR-T therapy can target specific B-cell populations with potentially deeper and more durable depletion, as demonstrated in exceptional cases, suggesting a path forward for precision targeting in select patients.

Q: Before we wrap up, two questions. There are patients reported who received Rituximab and developed colitis; what is your take on this and on the broader efficacy in IBD?
Rituximab showed limited efficacy in IBD partly because intestinal plasma cells down-regulate CD20, and broad B-cell depletion can cause unintended inflammation by removing protective regulatory components; patient selection and targeted approaches are key.
Relevance of cell-based therapies in IBD including hematopoetic stem cell transplantation and CAR T cell therapies
Q: How would you summarize the current safety considerations for CAR T cell therapies in IBD?
Short-term safety appears acceptable and may be better tolerated than HSCT, but long-term safety data are limited; monitoring for hypoglobulinemia and infection risk is important as experience accrues.
