Chemo plus IO is still a reasonable approach for many patients, but in older patients or those with high comorbidity burdens we consider single-agent immunotherapy if the disease burden is low and the patient would tolerate it better; however, most patients still receive chemotherapy with immunotherapy, balancing efficacy and tolerability.
