  • Commentary rather than critique

Interesting topics in cardiology reviewing findings from papers in the EHJ. However, it would be great if there was some more analysis and discussion of the limitations of these papers rather than just commentary.

Journal editorial: Computed tomography-guided ablation of ventricular tachycardia: how predictable are VT circuits?
Q: So next I would like to ask Professor Philipp Sommer, what is your perspective on this data set, particularly regarding its relevance to procedural strategy, efficiency and EP adoption?
This study is important because it shows VT ablation trials can be conducted prospectively and randomized across multiple centers, suggesting a potential shift toward anatomy-driven strategies in ischemic VT. The data imply that a predominantly anatomical, imaging-guided approach could simplify procedures and reduce time, while still recognizing that some circuits will require conventional EP verification. MRI remains a key future tool for scar delineation, especially in non-ischemic VT, and AI-guided isthmus detection may improve consistency across operators. Overall, the findings could drive broader adoption and generalizability, with a role for EP maneuvers not being eliminated but potentially reduced in select cases.
Journal editorial: Computed tomography-guided ablation of ventricular tachycardia: how predictable are VT circuits?
Q: Dr. Sciacca, please, can you give us a short summary of this paper?
The In Your Heart Trial was a randomized study conducted across 14 high-volume centers, enrolling 113 patients with prior myocardial infarction and clinically significant VT, comparing CT-guided VT ablation to conventional electro-anatomical mapping-guided ablation. The CT-guided arm used pre-annotated CT models to guide a linear lesion approach and did not require VT induction, while the conventional arm relied on substrate mapping and induced VT when needed. The trial found a reduction in total procedure duration with CT guidance, especially when strictly adhered to per protocol, with comparable safety and similar acute efficacy; at 12 months, VT-free survival was numerically higher in the CT-guided group.
Journal editiorial: Subvalvular aortic stenosis in adulthood: stable, manageable, but never simple
Q: Salvatore, what does this conqueror study really tell us about the natural history of subvalvular aortic stenosis in adults?
It shows a large-scale, long-term view indicating stability of the obstruction, with slow progression and excellent prognosis in mild cases, but also highlights persistent clinical risk—especially in those who had prior surgical repair—necessitating broader, lifelong management beyond just watching gradients.
Journal Editorial - Another piece of the puzzle: long term follow-up of conduction system pacing
Q: Any limitations of the study you can think of?
The study's retrospective nature limits it, but it provides long-term follow-up and is consistent with previously published findings.
Journal editorial - Has artificial intelligence proven stress perfusion cardiac computed tomography obsolete or more relevant than ever?
Q: What are the challenges and main limitations to implementing this technology in clinical practice?
The main challenges are workflow and operational efficiency in busy medical settings, which can hinder the adoption of stress CTP.
