Q: Hello, Dr. Vissing, from a clinical standpoint, why are young adults an especially important group to study in glomerular disease outcomes?
There has been little research on the adolescent and young adult population in glomerular disease, and this life stage presents unique medical and life-transition challenges that can affect disease management. Studying ages 14 to 25 helps capture the transition from parental care to independent management and may reveal distinct outcomes across key glomerular diseases.

Q: I wanted to switch gears just a bit because I know this will be interesting to our listeners, which is the topic of transplantation for these individuals. Tell me about your thoughts on transplantation and whether you see anything new on the horizon that might mitigate the need for transplant for these individuals.
Dallas criteria have evolved; more centers transplant ALD patients and the field is moving toward post-transplant support to prevent relapse and reengage patients in treatment. There is growing evidence that relapse with abstinence after transplant yields mortality similar to those who never relapse, suggesting a shift away from rigid sobriety timelines toward treating alcohol use disorder and ensuring post-transplant ongoing care.

Q: I'm going to start with just some basic stuff, Jessica. Why don't you tell me a little bit about the prevalence and use of alcohol, and why it's so hard to recognize alcohol-related liver disease when it seems like in something that's been around for decades, and maybe hundreds and hundreds of years, we still struggle with finding people until they're pretty sick.
Alcohol use is widespread, with about 80-85% of people over 12 having a drink at some point in their lives; most are not heavy drinkers, but a subset develops alcohol use disorder that progresses to advanced liver disease unless identified early. Recognition is hampered by siloed care and under-screening for alcohol use alongside liver disease, despite patients interacting with various parts of the health system.

Q: What have been the key developments in therapy and guidelines, and how do you approach biochemical targets in your patients?
Therapies evolved from UDCA to OCA and beyond, with guideline developments lagging behind cutting-edge data. The goal now is achieving normal alkaline phosphatase with normal bilirubin, and integrating staging with biochemical response to tailor second- or third-line therapies, including considering newer agents like IBAT inhibitors for itch and overall disease burden.

Q: Can you share how your background shaped your interest in cholestatic liver disease and what drew you to this field?
I discuss being influenced by mentors during fellowship, especially Dr. Marshall Kaplan, and how early exposure to large PBC/PSC clinics shaped my view of cholestatic disease as a core specialty, leading to a lifelong focus on translational research and clinical trials in this area.
