  • Breakin’ it down

    How things are broken down to help people understand shows true mastery of the craft. (Although we’re always learning). He definitely has it though. I’m currently in paramedic school but can listen and understand because either I’ve learned enough in class to understand / how he explains it. Also, it’s already helped in the field and in class.

Q: Is seizure prophylaxis a common thing? Not a common thing? Not even something that you think about? Where is your kind of standpoint on that?
Not something we commonly think about or not something we commonly go for. Anecdotally, I've never, I don't think I've ever done seizure prophylaxis for an ischemic stroke.

If somebody has a high NIH score and clear MCA syndrome without contraindications to thrombolytics, and it fits within the appropriate time frame, then I would opt for that.

My biggest things I want to know are her pupillary response, her GCS, and if she withdraws to noxious stimulation in all four of her extremities.

Q: How do you manage the cardiac arrest?
If a cardiac arrest occurs, we manage it in the prone position as best we can, as flipping a patient is unsafe and impractical during transport.

Q: What are you guys doing in the field when a patient's intubated, mechanically ventilated?
We assess the patient, do a PF ratio, and if it's 140-150, we consider proning the patient for transport. We discuss with our medical control and the receiving hospital's physician before proceeding.
