The main concerns have shifted from a singular focus on pulmonary aspiration to a more nuanced assessment of gastric contents and their clinical impact. While GLP-1s can increase gastric contents, the risk of aspiration is not as high as once feared, and decisions now hinge on timing relative to escalation phases, symptomatology, and the planned procedure.
