隨著醫療服務的可近性與醫療科技的提升，民眾就醫行為普及化，隨之而來的醫療爭議案件也與日俱增。根據國內學者研究推估，台灣每年可能有近八萬件的醫療傷害案件，但如此這麼多的醫療糾紛案件，會進到訴訟的案件只有一部分，而民眾在這當中的勝訴率更僅1成。這些數據背後反映出，有多少醫療爭議事件，就代表就有多少位「身心受創」、「心力交瘁」的病患、家庭與醫師。更不用提一旦進入醫療訴訟，長期且耗時的折磨。正因為醫療爭議具高度專業、個案化與多樣性，可能與檢查、診斷、治療、機構設備、知情同意等相關，很難有一致的標準。民眾... more
人口高齡化是全球永續發展的共同挑戰，尤其是對於醫療與長照體系的衝擊，但可能一個下午看下來，居家醫療最多只能看3-4個病人，從事居家醫療服務的效益也比不上開業的效益，要讓醫師踏出診間從事居家醫療服務，極具挑戰；病家也不一定信任居家醫療的醫師，需要培養信賴感。
即使在種種限制下，居家醫療整合照護計畫從105-109年至今，需求量仍以驚人的幅度大為提升，總服務人次約從105年的3.5萬人，快速攀升至109年的53萬人。其中，病家需求的大宗是重度居家醫療(約30萬)，其次是居家醫療(約19萬)，最後... more
隨著人口高齡化快速增加，年長者的醫療、照護需求已成為民眾必須面對的課題。為了改善健保財務困境與醫療資源有效利用，分級醫療是一條很重要的道路。然而，民眾可能存在，基層診所可以提供的醫療照護以及資源不如大醫院、或廟大和尚靈的思維。
據《全民健康保險法》第44條，健保署應訂定家庭責任醫師制度，以達到促進預防醫學、落實轉診制度，並提升醫療品質與醫病關係的目的。而健保署從2003年起，推動「全民健康保險家庭醫師整合性照護計畫」（簡稱家醫計畫），社區醫療群也應運而生。

根據台大近年的統計，有照護需求的台灣老年人有1/3長期使用5種以上的藥物。在失能老人則有高達8成長期使用5種以上的藥物，甚至38%使用藥物高達10種以上。而2018年健保署的資料也顯示，多重慢性病人平均用藥7到8項。這麼多藥往往都不是同一醫師、科別所開，也可能衍生複雜用藥、多重用藥或交互作用等問題。

根據健保署歷年統計，全台約200多萬名糖尿病患者，每年增加約7萬名的糖尿病人數，但根據2018國際糖尿病聯盟大會發表的「台灣糖尿病登錄計畫」報告，卻有超過6成、超過120萬名的糖尿病患者血糖控制未達標。但中華民國糖尿病學會曾多次表示，糖尿病治療重點為控制血糖，若未妥善控制，會引起許多併發症、多重共病，不只影響健康，同時提高醫療費用，還會增加患者的經濟負擔。
