Q: What were the main obstacles to implementing telemedicine-based ROP screening and how were they overcome?
Key challenges included limited monitors and equipment, the need for trained personnel, and coordination across facilities. We established a PACS-based telemedicine system with trained community health workers supported by Harvard and Stanford ophthalmologists, created dedicated screening days, and expanded to multiple sites with ongoing NGO support to cover costs and logistics.

Q: Could you walk us through how caffeine citrate was introduced and what the national process looked like?
The process involved collaboration between the Ghana Health Service, the Ministry of Health, CHI, and external partners. A technical working group drafted National Guidelines on Apnea of Prematurity, CHI donated caffeine citrate to start the program, and regions were trained under a trainer-of-trainers model. The aim is to have caffeine citrate included in standard treatment guidelines and emergencies drug lists so NHIS can cover it.

It started as the Neonatal Intensive Parent Club born from the need to educate and support overwhelmed parents; over time, it expanded into a formal charity with governance, fundraising, and ongoing involvement from families and nurses.
