Talking Points

Recent interactions between the hosts and their guests.

Emily Dodwell and the fragility of the science in orthopedic surgery
Q: What is the fragility index and what did your pediatric orthopedic study reveal about it?
The fragility index measures how many patients would need to change outcome classification to alter a trial's result; in pediatric orthopedics, the average fragility index is around three, meaning small changes could flip conclusions, which raises concerns about reliability of small trials and the need for more robust evidence and better reporting practices.
Emily Dodwell and the fragility of the science in orthopedic surgery
Q: Can you explain what parent-derived health utility scores are, and why they matter for DDH management?
Health utilities are scores that quantify how desirable or burdensome a health state is, on a 0 to 1 scale, based on parental perception for states like wearing a Pavlik harness, a spica cast, post-surgical recovery, or end-stage arthritis. They enable cost-utility analyses across a patient's lifespan to compare different screening and treatment pathways.
Mike Vitale & Safety in Surgery
Q: What role do metrics and specialization play in improving safety?
Metrics like a specialization index help align teams, communicate with administration, and drive consistent collaboration among clinicians, leading to more reliable, low-variability care.
Mike Vitale & Safety in Surgery
Q: How do you balance adopting new technologies with the obligation to not introduce new failure modes?
Approach innovation thoughtfully, measure outcomes, engage diverse teams, ensure informed consent, and rely on best practices and governance to avoid premature or unsafe adoption.
Mike Vitale & Safety in Surgery
Q: What is the central idea behind building a culture of safety in spine surgery?
Safety starts with treating it as a system design issue, using standardization, checklists, and psychological safety to ensure team members speak up and shared learning reduces avoidable harm.
