Q: So, should we proactively screen for AIN in all people with HIV, including people assigned female at birth?
Screening should be targeted toward high-risk groups first, with broadening as evidence and resources allow. Gay and bisexual men and trans women with HIV are prioritized, but guidelines suggest inclusion of people assigned female at birth who have HPV-related disease in the lower genital tract or have had solid organ transplants, with a careful consideration of risk versus cost and resource availability.

Q: What is your vision and what needs to be done to create sexual health services that suit trans and gender diverse people better?
There is a need to stop conflating transgender women with men who have sex with men in research, and to implement tailored interventions and targeted funding and policies that address trans-specific HIV/STI risks and barriers to PrEP access, with greater involvement from community-led, multilingual approaches.

Q: Could you tell us how your work started and what discoveries you have made along the way?
The work began with recognizing a high prevalence of HIV in transgender women, up to about 20%, with STI rates also higher than in the general population. This prompted targeted actions to improve PrEP access and STI prevention, including outreach beyond the hospital to reach women who avoid clinical settings.

Q: Can congenital syphilis be eliminated, and what would it take to reach that goal?
Elimination is possible in theory, but requires political will, public engagement, sustained funding for testing and treatment, stronger antenatal programs, and expanded access to rapid testing and surveillance, as demonstrated by successful campaigns in Brazil and other regions.

Q: How common is congenital syphilis, and is the incidence changing globally?
Incidence has been rising worldwide since 2000, with significant increases in countries like England (e.g., over 13,000 new cases in 2024 vs. around 5,000 in 2015) and notable rises in Australia and Brazil, indicating a growing public health challenge.
