Q: Why did you go for Nortriptyline? Why not amitriptyline or another neuromodulator?
Nortriptyline was chosen because it has fewer anticholinergic adverse events than amitriptyline, and its simpler metabolism (single enzymatic step) facilitates a more precise, personalized dosing approach through preselection based on genotype.

Q: Then, in my opinion, one of the most interesting parts of your study is the role of expectations on symptoms, the expectations towards the treatment effect. Can you elaborate a little bit more about that? What did you find, and how can we use it maybe in clinical practice even?
The belief about which treatment participants thought they had received was significantly associated with symptom response, implying expectancy effects are a real driver of outcomes in functional dyspepsia. Clinically, this suggests framing treatments positively and realistically may enhance outcomes, and that clinicians should consider patient beliefs as part of treatment planning.

Q: What are the practical implications for clinicians treating post-infection IBS?
Clinicians should identify whether IBS symptoms began after infection or antibiotic use to tailor prognosis and consider microbiota-targeted therapies (diet, probiotics, non-absorbable antibiotics) and be cautious with antibiotics to avoid unnecessary exposure; standard IBS treatments remain applicable but with attention to diarrhea-predominant phenotypes and potential anti-inflammatory approaches in selected cases.
