  • Essential for residency

As an ENT intern I find this exceptionally useful for general studying and preparing for clinic and cases. I use it as a pre-reading but often times it feels like I learn so much from it alone that I do not need to do a deeper dive.

  • Really clear and high quality produced information

I’ve only listened to 1 episode so far (the one on human centred design and implementation), but I already feel hooked. The host is incredibly clear and thoughtful in their questions. Really appreciate the episode on this topic because I’m recently entering this field of work and podcasts are so important for my self education. Thank you.

If possible it would be good to produce some show notes for the previews.

Talking Points

Recent interactions between the hosts and their guests.

Complications in Otologic Surgery: Avoidance and Management
Q: What is the initial consideration for a clinician with a patient who wakes up from otologic surgery with facial paralysis?
The first considerations include reviewing the surgery to determine if there was difficult dissection near the facial nerve or if the nerve monitor activated. If the operation was clean, observation is recommended as there might be a temporary paresis from local anesthetic. In cases of mild weakness, observation may continue, but severe weakness may necessitate returning to the OR for surgical decompression.
Complications in Otologic Surgery: Avoidance and Management
Q: What are the general rules of thumb to prognosticate recovery of facial nerve injury after iatrogenic damage or trauma?
Two rules of thumb that can be used to prognosticate facial nerve injury are one, time to injury with a more delayed onset to facial nerve weakness, portending a better prognosis and chance for recovery. The second is the nadir of the facial nerve function, where a lower House Brackman score indicates a better prognosis.
Tumor and Tumor-Like Benign Oral Cavity Lesions
Q: What are the boundaries of the oral cavity?
The oral cavity begins anteriorly at the mucosal or wet lip and extends to the junction of the hard and soft palate above and the line of the circumvalid papillae of the tongue below.
Facial Nerve Tumors
Q: What are the three major management modalities for patients with facial nerve schwannoma?
Management involves microsurgical resection, stereotactic radiosurgery like gamma knife, and observation for small to medium tumors with good function.
Facial Nerve Tumors
Q: Could you provide some more specifics on to the types of primary nerve sheath tumors that we encounter when looking at the facial nerve?
Broadly, this episode will primarily focus on facial nerve schwannomas, but also includes discussions on neurofibromas and geniculate hemangiomas.
