    I’m a PGY-2 resident and have been listening this podcast for two years now (dating back to MS4 days). With that said, I believe there is invaluable education here with these monthly podcasts, and this led me to think about of the concepts presented in ways myself that I otherwise would not have.

    The Q&A format opens up to a deep dive into concepts that simply reading the articles myself would not have obtained.

Q: Peter, you noted that the lack of a comparator group limited the external validity and generalizability of your investigations findings. If you were to conduct a comparative study, what would be your ideal comparative cohort to the fourth generation percutaneous transverse osteotomy and why?
I would choose to compare it to the Lapidus procedure to see if there's a difference in pronation correction and to single screw fixation to investigate stability improvements.

Q: Tom, prior to your fellowship with Dr. Lam, I trust that you had some experience with open hallux valgus surgery. Aside from the obvious differences in exposure, what do you consider to be the key advantages of MIS hallux valgus correction over traditional open procedures?
Two things where MIS is better are in the early recovery with patients back in normal footwear earlier with less pain, and potentially lower long-term recurrence rates due to more bony corrections.

Q: Peter, your investigation focuses on the fourth generation of percutaneous transverse osteotomy. For our listeners who may not be as familiar with the different generations of MIS bunion surgery, would you briefly describe the evolution of the percutaneous transverse osteotomy technique?
The second generation was a transverse osteotomy with KY fixation. The third generation introduced a chevron osteotomy with twin screw fixation, but these methods can compromise stability, which the current technique aims to avoid.

We did a retrospective study from a single centre of 51 feet from 34 patients who underwent weight-bearing CT both before surgery and after surgery, and we found significant improvements.
